Getting to Know You
Month 1
- Fill out health questionnaire and tell us about your main health goal
- 30 minutes comprehensive video visit with your doctor
– In-depth counseling of your lab test result and behavior patterns
- 30 minutes comprehensive video visit with your health coach
– Review your lifestyle across diets, sleep, exercise, and more
- Get your blood test* done at a lab near you after the 1st doctor visit
*The cost of laboratory tests is not included in membership,
but many tests can be covered partially or fully with your health insurance.
1st Cycle
Month 2
- 20-minute kick-off meeting with your doctor
- Set specific and achievable action plans with your health coach
- Record your health data and food diary on the patient portal
- Discuss the best medication for your chronic condition with your doctor
Month 3
- Optimize your action plans on diets, sleep, stress, and activity on a bi-weekly basis with your health coach

Month 4
- Get a quarterly lab testing to check on your progress
- Review your health status with your doctor based on your Lab results
- Adjust your behavior and diet change plans with your health coach
2 - 4th Cycle
Month 5 - 1 Year

DoctorHere doctor and health coach visits take place over video calls, and you can always reach our Care Team through Secure Messages from DoctorHere Portal. That means you can access DoctorHere from your home, or anywhere else you happen to be.

When you join DoctorHere, you’ll provide your health information, including blood work, that our Care Team will review before customizing your program.
