Membership

DoctorHere Membership is a data-based behavior-changing chronic disease care program.

DoctorHere members provide their health goals, preference, behavior pattern, and blood test results upon joining DoctorHere. Based on the data members provide, our Care Team and each member work together to devise a personalized care plan. Each care plan consists of missions called action items that a member needs to carry out through their membership cycle helping form healthy habits. DoctorHere Care Team will follow up with each member regularly throughout the cycle to address issues while keeping members accountable. Our primary focus will be geared towards normalizing abnormal health data (such as your blood pressure, blood sugar level, weight, etc) with doable action items. The ultimate goal of our membership program is to empower our members to be able to better take care of themselves on their own by the time they are finished with our program. We work closely with our members as they gradually learn more about themselves and figure out what works best for them. We call this becoming self-empowered with DoctorHere.

DoctorHere membership includes:

  • Recurring virtual visits with the doctor
  • Biweekly virtual visits with a health coach
  • Need-based virtual visits with doctor or health coach if the patient needs extra care
  • Access to patient portal and its features:
    • Receiving and viewing personalized care plans
    • Logging vitals and action items
    • Online billing and scheduling
    • Access to advanced biomarker testing
    • Unlimited messaging with your care team through an online patient portal
    • Access to prescriptions for medications and supplements
 

What’s Not Included:

  • Cost of laboratory tests. Many tests are covered partially or fully with insurance. However, some may require a co-pay or may not be covered at all
  • Medications (prescription and over-the-counter) and supplements

Our memberships can be paid monthly or upfront, more details on pricing can be found here. This cost covers the comprehensive care and unlimited messaging you receive from your doctor and health coach throughout your membership. 

Lab work and prescriptions are not included in the cost of your membership.

DoctorHere Care Team consists of three people. 

Doctor: Doctors oversee the entire health journey of each member. They make sure each patient under their care is headed towards the right path and prescribe lab and prescriptions as necessary. 

Health Coach: Health coaches support doctors in guiding members and act as their main communication point. Members will meet with them biweekly to share their progress and touch on points where improvements can be made.

Care Manager: Care managers mainly handle administrative tasks related to members’ care such as scheduling appointments and labs, answering and triaging secure messages, facilitating the billing process, etc. Members can always reach out to Care Managers to address their concerns and questions.

No. Each individual over 18 years old must create an account under their legal name.

If you have a child who needs our service, you must create an account on behalf of your child and be present when the child is seeing our Care Team.

Our doctors can prescribe a wide range of drugs, which can be useful for treating chronic diseases, infections, allergies, and other health concerns. 

DoctorHere does not prescribe elective medications, certain anticonvulsants, narcotic pain relievers, or other drugs listed as controlled substances by the U.S. Drug Enforcement Agency or regulated under State law. 

Review the DEA Controlled Substances Schedule if you are interested in a particular prescription and are uncertain if it is listed as a controlled substance.

An online visit is just like an in-person doctor or health coach visit except you hold it through a secure video platform. You will receive an email with a link to open a video screen where your doctor or health coach will be waiting to talk with you.

Members can directly message their DoctorHere care team through a private member portal. If a question or request is deemed too complicated to be addressed through messaging, our Care Team may ask you to schedule a visit. Note that we typically respond within 24-48 business hours during the week.

Many of our members use their FSA/HSA to pay for membership fees, lab tests, and prescriptions. We recommend checking with your FSA/HSA company/carrier for more information and details on how you can use those funds. We can also provide a superbill if necessary. In case there is any additional documentation you need, we are happy to assist. If you have any other questions, please reach out through Secure Messages from our patient portal.

No. While we do offer primary care on need bases, our membership is mainly focused on chronic disease care. DoctorHere is a complement to—not a replacement for—your primary care provider. While we look forward to partnering with your primary care provider and other specialists to share your labs and updates on your progress, it is recommended that you maintain your relationship with your primary care provider outside of DoctorHere.

Program

Yes. Our program is entirely digital using telemedicine to treat you safely and effectively. DoctorHere doctor and health coach visits take place over video calls, and you can always reach our Care Team through Secure Messages from DoctorHere Portal. That means you can access DoctorHere from your home, or anywhere else you happen to be. 

Your DoctorHere Care Team uses your lab results, daily tracking measurements, and health history to treat you. When you join DoctorHere, you’ll provide your health intake, including blood work, that our Care Team will review before customizing your program. It’s important that you provide our Care Team with accurate information in your assessment and your telemedicine visits so our Care Team can provide the most accurate and customized care for you.

DoctorHere provides customized missions for each member to carry out called action items. Such action items can include but are not limited to:

– Establishing a sustainable and flexible approach to nutrition

– Logging your daily measurements

– Exercising 30 minutes every day

– Making food healthier choices that include protein, fiber, and healthy fats

– Writing a food diary, stress diary, sleep diary

You and your Care Team will work together to come up with actionable items just right for you in your 1:1 virtual sessions. You’ll be able to view and log your action items from the DoctorHere Portal.

After the first month, you’ll be mostly meeting with your health coach for 15 – 20 minutes every other week, and carrying out action items that are assigned to you at these meetings. In addition, there are periodic doctor visits and lab visits to track your progress. There will also be asynchronous communication through secure messages where your Care Team will request specific actions from you and you can also ask any questions to your Care Team. However, you as a member ultimately control when you do all of these things.

All fasting and timing considerations for each lab order will be specified by your DoctorHere physician within your most recent Encounter History. If you need to request any changes to your lab order, please message your Care Team at least 24-hours before your scheduled lab visit. We cannot guarantee that same-day changes to your testing will be available.

Before your blood draw:

– Drink plenty of water both the day before, and the morning of your lab draw to ensure you are well hydrated ahead of your visit

– If fasting is required: please do not eat, drink coffee, or tea for 8-10 hours before your visit.

– Discontinue any vitamins or minerals 48-72 hours before the lab draw unless otherwise specified by your clinician

– Continue prescription medications unless otherwise specified by your clinician

– Avoid intense exercise 24-hours before the lab draw.

What to bring:

– A copy of your insurance card and photo ID

– Bring a printed copy of your lab order.

After your draw:

– If you feel dizzy or light-headed, hang around until you feel better, and drink water to hydrate.

– Avoid strenuous exercise for a minimum of 4-hours after your lab draw.

Call 911 immediately in the case of a medical emergency. DoctorHere is not intended for emergency or urgent medical care. If your concern is not severe enough for 911, please contact your primary care provider or other managing specialists for urgent attention.

Log into your patient portal using your mobile or desktop device. Go to ‘My Vitals’ tab once you log in and record your progress as you were assigned by your Care Team.

You don’t have to worry about making appointments with your providers as we will reach out to you when it’s time for your next appointment. Your recurring health coach visits will be scheduled at the end of every meeting. If you’d like to schedule additional visits outside of your regular visits, please reach out to your Care Team through Secure Messages and include reasons for your appointment request.

Pricing & Insurance

To learn more about how to better understand your coverage before moving forward with testing, we recommend speaking with a representative from your plans directly. To find out which lab is in-network with your plan, you can either call the Member Service’s number printed on your insurance card, or log in to your insurance provider’s member portal to find out which labs are preferred in your plan, such as Quest, LabCorp, or BioReference.

Depending on your coinsurance and deductible, lab testing sometimes requires a co-pay or out-of-pocket fee. Coverage can also depend on your specific insurance plan, diagnoses, and whether the lab is in-network. If you’d like to better understand coverage before testing, we recommend speaking with a representative from your plan directly as many members find it helpful to ask a few of the following questions:

– What percentage of in-network diagnostic laboratory services are covered by my plan?

– What is my preferred in-network lab provider? Can I complete testing at BioReference, Quest, or LabCorp?

– What is my current deductible status?

– What coverage can I expect for the following testing? Ask your DoctorHere Care Team for a CPT document for your specific lab order, which will include both the billable CPT and ICD-10 diagnosis codes associated with your order. By providing these codes, a representative will be able to advise which tests are eligible for coverage based on your plan. Upon request, this CPT document will be uploaded to your DoctorHere Portal’s ‘Labs & Document’ page labeled accordingly.

Many of our members use their FSA/HSA to pay for clinical fees, specialty tests, and supplements. We recommend checking with your FSA/HSA company/carrier for more information and details on how you can use those funds. In case there is any additional documentation you need, we are happy to assist. If you have any other questions, we’re always just a message away on your DoctorHere Portal.

The cost of testing is not included in your membership fee. All testing costs are the patient’s responsibility. Testing recommendations are completely optional and can be personalized to your budget. We work with you to review your testing options and financial comfort and move forward with the tests you choose. Depending on your insurance plan, you may receive partial or full coverage for blood work or specialty tests. Many members use their FSA/HSA to contribute to the cost of specialty tests. Please do not complete testing without exploring cost and coverage directly with your insurance company, as the tests we order can be comprehensive depending on your condition.

A DoctorHere membership does not replace health insurance. We advise all members to maintain a separate health insurance plan of their choosing. For blood work, insurance frequently offers full coverage.

Lab tests and prescription medications are not included in the membership cost, but we’ll work with your commercial/employer insurance to get them covered if applicable.

Yes, your membership will automatically renew at the end of each subscription period. You will receive reminder emails before each charge. If you’d like to cancel your membership, please speak to your Care Manager.

Privacy & Security

Yes, as a healthcare company, we take member privacy very seriously and operate in accordance with medical law. We are HIPAA-compliant and have designed our platform to comply with all relevant privacy laws. We have implemented strict security protocols to protect your information. Please read our Privacy Policy for more details.

This will vary from state to state as each state law has different requirements. We retain medical records in accordance with these laws.

HIPAA is the Health Insurance Portability and Accountability Act. This act is the national standard to protect sensitive patient health information from being disclosed without the patient’s consent or knowledge. It also includes the HIPAA Security Rule, which regulates the storage and protection of all PHI.

PHI stands for “Protected Health Information”. This is an acronym used in Federal Health Privacy Rule and includes any and all personal information, both relating to health and identity.

My Account

Navigate to ‘My Pharmacy’ tab from your DoctorHere Portal. You can remove a pharmacy from the list, change your primary pharmacy, or add a new preferred pharmacy using the buttons provided.

Go to ‘My Account Details’ from DoctorHere Portal. You will be able to update your address here.

You can contact our customer support team in two different ways: You may send a message through your DoctorHere Portal. Log in here (doctorhere.md-hq.com) and navigate to your Secure Messages tab. Press ‘Send a New Secure Message’ then choose your Care Manager to receive your message. You may also email our Support team directly, via support@medihere.com. Please include your full name, email address, and phone number linked to your account along with the content.
